1. Notice of nondiscrimination

Jones Medical, LLC complies with applicable federal civil rights laws and does not discriminate on the basis of race, color, national origin, age, disability, or sex (including pregnancy, sexual orientation, and gender identity).

Jones Medical does not exclude people or treat them differently because of race, color, national origin, age, disability, or sex. This applies to who we accept for care, the care we deliver, how we schedule visits, who we assign to a visit, and how we handle complaints.

We also do not discriminate on the basis of religion, marital status, veteran status, genetic information, or source of payment.

2. Free aids and services we provide

Jones Medical provides the following at no cost to you:

2.1 For people with disabilities, to help you communicate with us

• Qualified sign language interpreters
• Written information in other formats, including large print, audio, and accessible electronic formats
• Assistance with reading and completing forms
• Communication through the Oklahoma Relay Service — dial 711

2.2 For people whose primary language is not English

• Qualified interpreters, including by telephone
• Information written in other languages
• Verbal translation of documents, including your plan of care and consent forms

Please do not rely on a family member or a child to interpret for you. You have the right to a qualified interpreter, and we will arrange one. Ask any Jones Medical caregiver, or call (405) 907-8884.

3. How to file a grievance with us

If you believe Jones Medical has failed to provide these services, or has discriminated in any of the ways described above, you may file a grievance with our Civil Rights Coordinator.

You may file in person, by mail, by fax, or by phone. If you need help filing a grievance, our Civil Rights Coordinator will help you — including by writing it down for you if that is what you need.

We will acknowledge your grievance, investigate it, and give you a written response. We will not retaliate against you for filing, and filing will not affect the care you receive.

4. How to file a civil rights complaint with the government

You may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights. You may do this instead of, or in addition to, filing with us.
